Completing Your Secondary Application
Before completing your child's application to secondary school, please ensure that you have read the "Secondary Admissions Guide"

If you are having difficulty completing the application online, please contact Mr Wall, who can support you completing your child's application.
If you are unable to complete the online application and need to complete a paper application, you can download an editable copy and email the completed form to [email protected] or post it to:

If you are applying to Maiden Erlegh or River Academy and your child is in receipt of Free School Meals, or they have been in the last 6 years, you should also complete the supplementary information form. This form provides the schools with information on their Free School Meal and Pupil Premium status. Please click the links below to download the correct form:
11+ Assessments - a note for parents
We strongly believe that every child should have the best possible education and fully support families who choose to attend assessments for the 11+ exams and Grammar School entry. However we also hold high expectations for children attending Civitas and this means that only the time required to complete the assessment will be an authorised absence from school. If parents keep children home for more time than needed to complete the assessment, the time will be recorded as unauthorised.
Parents should inform the school as soon as they have a booking to complete an assessment and provide evidence of the booking. Notification should be submitted using the absence from learning form or by requesting a paper copy through the school office.
